Your Visit


We ask our patients to arrive 15 minutes before their scheduled appointments to allow time for any updates to patient files and other necessary paperwork.

If you must cancel your appointment, we’ll be happy to reschedule at a more convenient time. We ask that you give us 24 hours advance notice, providing an opportunity for another patient to be seen.


What to bring to your appointment: 

  • Photo ID
  • Current insurance card(s)
  • Co-payment(s), if required
  • Tests results/films that have been requested by us
  • Past medical records you feel are applicable to your health
  • If you have Medicare, your pharmacy insurance drug card

If you would like another person to have access to your OhioHealth MyChart account, please complete this proxy form and bring or mail it to our office.
